The Internal Revenue Code has several complex rules relating to the taxation of retirement plan distributions. Below is important information and the forms shareholders will need to request a distribution.
Taxable distributions are subject to federal tax withholding based on the type of retirement account and type of distribution elected. Shareholders should review the Mandatory Election for Tax Withholding section of the appropriate form(s) below.

IRA Distribution Request Form — Request a distribution from a Traditional IRA, Roth IRA, SEP IRA, SIMPLE IRA or SARSEP for which Fiduciary Trust International of the South ("FTIOS") serves as custodian.
Beneficiary Transfer/ Distribution Request Form — Request a transfer and/or distribution of retirement assets to the beneficiary of an IRA, Roth IRA, SEP IRA, SIMPLE IRA or SAR-SEP account for which Fiduciary Trust International of the South ("FTIOS") serves as custodian.

Putnam IRA Distribution Request form
Request a complete, partial, or systematic distribution from a Putnam Traditional IRA, Roth IRA, IRA rollover, Roth IRA conversion, SEP, SARSEP, or SIMPLE IRA account.
Putnam IRA Beneficiary Distribution form
Request a transfer and/or distribution from a Putnam IRA inherited from a deceased account owner to a non-spouse beneficiary or a Putnam Beneficiary IRA inherited from a deceased account owner to any type of beneficiary.
Putnam IRA Beneficiary Distribution form for a spouse beneficiary
Request a transfer and/or distribution from a Putnam IRA inherited from a deceased account owner to a spouse beneficiary. Do not use this form to request a transfer/and or distribution from a Putnam Beneficiary IRA Inherited from a deceased account owner.
